To be honest its the basics.
Eat healthy (some cripsps won't hurt now and again) - !Have a 8 hour + sleep! - No drugs / excessive alcohol - Some daily excersices are good!

Just cut out junk food and you will feel a big difference just from that.

I would also like to add in hygiene as a step to a healthy life.
Always make sure to clean properly in the shower using the right products such as shampoo and body wash. Also, the smallest of things will help including the trimming of your nails regularly.

1.Wake-Up Early
2.Set a Goal
3.Schedule a regular workout time
4.One simple way to stay motivated is to look back on how far you've come.
5.Give yourself a reward! Sometimes the 'big picture' can be too intimidating.

Sleep on time. There is a fallacy that staying up late would make you skinny; your body would actually stack up on energy (in the form of fat).
