My friend was riding his bicycle. He got hit by a car. He is in the hospital now. Should he pick a general personal injury lawyer in his hometown? Or should he pick a personal injury lawyer who specializes in bicycle injuries even if the lawyer lives hundreds of miles away?

Local would be cheaper and more personal, but you'd have a lower chance of winning a court case.
Specialised would be the opposite.

Really depends if you want to pay extra.

Your friend should hire a good personal injury lawyer. But that lawyer should have a good record and experience to deal with such type of accident cases. You don't need to hire any other lawyer. A personal injury lawyer can handle your case easily.

Personal injury lawyers also deal with the injuries caused by accidents. But you should select good injury lawyer who has well knowledge in his area and has enough experience to handle such cases.
