I'm guessing make ur sig really small on the little line. Than put the date on the big line ?
Or you put ur name on the big line, and put the date under the word date.

It is difficult to tell without knowing the purpose of the document. My assumption is that two people are required to sign it:
Per: [ signing officer for X ] authorized signing officer for:[ X ] If it is acceptable for only one person to sign it, then your example does apply: Per: My Signature Authorized signing officer for: My Initials

Yes, two people are meant to sign it, but there are two of those blocks side by side.

If the instructions are not clear, all you can do is sign it as you see fit.
