You copied your entire code out here when all you needed was this i'm assuming:

What do you mean by separate? Just freely separate from the main form? Use Show() then instead of ShowDialog(). There's not much here to leach though so I wouldn't worry about that (no offense intended). Most of this stuff is just the designer code, link clicks (majority), and a couple button click event subroutines.

For input you could do InputBox.Show(). It returns a string so make sure and declare one for the returned value.

InputBox doesn't have method Show(), the parameters are specified directly from the InputBox function keyword. And you don't have to make sure that you declare a value as long as you check it's returned value before using it for a null value if you want some possible "error" handling for what you don't want it to do afterwards if there's a wrong value being returned.
