Depends what game. Normally they just code their own file, and then insert it, or simply create their own 'flavor' of the game.

They mod for the last rank because everyone wants to be that, thats why you play the game. After you get the highest rank it feels good to brag.

Modding games is actually pretty easy. Since it runs on your computer Windows messages get sent in to your system just like anything else. Animation on your taskbar, your start menu, explorer navigation, etc... And if you find out which windows message gets sent, you can hook it and either block it, or send a message of your own using the Win API (user32.dll).
Nothing truly special about modding games, but sometimes it can be fun if you need to cure some boredom. Even the process of making a little cheat engine is pretty fun

I mod games because I get bored of playing the same thing over and over again. So, I try to create something new.
People are sick of having to play the same level over and over again. This just gets really boring and uninteresting. You play several campaign missions several times and get tired of playing it. So you decide to modify the level and unleash some things you could not do. This is why games are popular: People are able to make their own version of levels and you can do things that you wish you could do.
Modifications that does not involve hacking or cheating and works for everyone should be allowed on any online game, including Xbox Live. I've dreamed about me and my friends playing Halo and fighting humans and Spartans. Sucks that that can't happen.
Again, People mod because they think playing the same level over and over again has gotten old. So they modify the maps and do things they want to do.
The point of modding is that so the modder can have fun and enjoy themselves with their new versions. Nobody should be taking that ability away.
