Maybe you wrote incorrect Payee name.
You either entered wrong info or maybe you just need to upgrade what you have written.
Google Adsense can be a pain.
I've not had a successful past with Google and it's ad sense or anything ad related. I suggest you talking to Labrocca(Omniscient) On his forums he uses it.
I've added the code and tried it but it was not successful with me.

1 Question, are you from India?
Cause Adsense has become strict from India because of the lot of scams from it.
If you really want to get approved, then atoztopmp3@gmail.com guy does it.
Even i have done from him and nothing problem occurred till now.
Does adsence not pay you crap money unless thousands view/click the blog ?
I got rejected by google adsense sometime back, since then I haven't applied
